Our Mission
At All 4 Me and Mommy PNCC we have a mission to provide exceptional services to all mothers we work with and connect them with resources that they can utilize to provide the best care for themselves and their children within the community we serve. Our care coordinators are here and ready to help with your journey throughout pregnancy and motherhood after. Our services will help guide mothers to accomplish goals set for themselves to keep themselves healthy and safe, help with CBRF training, resume building, education, and so much more.
Services we offer
Prenatal care coordination services include:
- Help getting clients the health care they need
- Personal support
- Information on good eating habits and health practices
- Help finding needed services in their community
- The purpose of the Medicaid Prenatal Care Coordination program is to provide access to medical, social, educational, and other services to pregnant women who are considered high risk for adverse pregnancy outcomes. The components of this benefit are outreach, assessment, care plan development, ongoing care coordination and monitoring, and health education and nutrition counseling. Some key outcome indicators include: Tobacco Exposure, Alcohol Use, Breastfeeding, Safe Infant Sleep Practices, Perinatal Depression, Family Planning, and Involved Father.
Childcare coordination services include:
- Improving family functioning
- Improving parenting skills and positive parenting outcomes
- Increasing recipients understanding of infant and child development
- Increasing recipients access to and appropriate use of the health care delivery system
- Improving employment outcomes
- Encouraging planned pregnancies
- Improving future birth outcomes
